I would say it is NOT a patch. The boxed copy of the game mentions only an online activation it does not advise of extra download.
I've seen a lot of speculation regarding if the day 1, 5GB Wolfenstein patch will be mandatory before being able to play the game.
So I shot off during my work lunch break and picked up my pre-ordered physical copy of the game. Quickly went home, chucked the disc in and after a minute (literally) of preparation, the game campaign could be started. And then i dragged my sorry self back to the office....

As you can see in the images, it notified me of the update which was required to access network features, but did not impact the ability to begin the campaign.
So no need to worry for those with slow internet connections (like mine), or temporarily without, or just wanting to get into the game. It won't be a problem.
TL:DR: Day 1 Patch is not required to download before playing the game
